Service Description

Termly Art Sessions - This is for the entire term including all 12 sessions. This is class for Tweens from ages 11 - 16 years of age. This is a class for kids who have a love for art but don't get the opportunities to do enough at school. The focus will be across specific projects like focusing on sculpture, specific artists and their disciplines to really focus and hone in on developing projects to be proud of. From researching and understanding concepts, to planning, to finally getting hand ons to create their final projects we will be focusing on a huge array of mediums, styles and projects. Our first projects for the term will be on sculpture. Understanding artists, the use of materials to create 3d objects, carving and using tools to hack and shape this will be an exciting project to get the kids stuck in! This is for the Wednesday Session. Cancellation Policy

In reading this document I acknowledge and represent that I have read and understand it; that I read it voluntarily and for full and adequate consideration, fully intending to be bound by the same; and that I am at least eighteen (18) years of age, fully competent, and the legal parent or guardian of my Child. As the parent or legal guardian of the minor child named in this booking (“my Child”), and in exchange for the benefits to be derived by my Child’s participation in the art class mentioned in this booking (the ”Activity”), I agree, on behalf of myself and my Child, to the following: 1. I give my permission for my Child to participate in the Activity. 2. On behalf of myself and my Child, I voluntarily assume all responsibility for any risks of loss, property damage or personal injury (other than personal injury due to our negligence) that may be sustained by my Child, or any loss or damage to property owned by myself or my Child, as a result of my Child being engaged in the Activity. 3. On behalf of myself and my Child, as well as our respective estates, heirs, administrators, executors, and assigns, I hereby release, waive, discharge and covenant not to sue Adrienne Konviser or any related parties, from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by me, or my Child, or to any property belonging to me or my Child, while participating in the Activity. 4. It is my express intent that this specific declaration shall be deemed to be a RELEASE, WAIVER and DISCHARGE for Adrienne Konviser and/or any related parties. 5. HEALTH CARE AUTHORISATION: I hereby authorise Adrienne Konviser to perform any acts which may be necessary or proper to provide emergency health care to my Child in the event that neither I nor the emergency contact can be reached, including consent to and authorisation of medical procedures by qualified doctors hospital or other emergency medical personnel, as they, in the exercise of their profession and in their sole discretion, may deem necessary. I understand that I will be responsible for all costs and expenses (if any) of such medical treatment.
